Thank you for electing to host a table at this years annual banquet.

• The average table seats (6) guests. There are limited numbers of larger capacity tables that will be filled based on the time of registration ( first come, first serve). We will do our best to seat larger groups as close to possible should they exceed the standard .

• This form will allow you to register as a host, make payment and include your guest names. Should you not have your some or all of your guest list yet, please continue with the form and leave the guest name details blank. Upon completion of the form you will have access to a link to return to the form to edit your submission. ​
